Today, Harmonic Health, a technology-driven dementia care platform, has officially announced its launch in conjunction with a strategic partnership with Stoney Batter Family Medicine, a medical practice with multiple locations across Delaware, Maryland, and Pennsylvania.
Harmonic Health, headquartered in St. Louis, is one of the innovative enterprises incubated at Redesign Health, a company dedicated to nurturing, launching, and securing funding for digital health startups.
Harmonic Health presents a holistic approach to care management for patients and their caregivers. This includes the provision of a physician-engagement platform, as well as round-the-clock behavioral, social, and medical support. In addition to these critical services, Harmonic Health also delivers telehealth solutions, predictive care offerings, and educational content to enhance the overall care experience.
In partnership with Stoney Batter Family Medicine, Harmonic Health will initiate a program aimed at delivering dementia screening, caregiver support and guidance, education, and referrals for community services to physicians and their patients.
